The actual Proof of Purchase Form is at the end of this document. To make sure you submit your form correctly, you'll need to read the following --

START HERE - THIS IS CRITICAL INFORMATION

Your Proof of Purchase Form must be submitted before you purchase Volume 14 or within 20 minutes of purchasing Volume 14.

We realize that asking you to submit your Proof of Purchase Form before buying Volume 14 is a little confusing. You may recall that when we first released the books, we said that the Proof of Purchase Form would be submitted after Volume 14.

Here’s what happened.

As the fervor grew for the 14-volume set, in part because of the material being exhumed from J. Michael Straczynski’s vault for Volume 15, we revisited our original plan with the help of an expert. We were advised that the best way to protect the investment of the fans was to ship the free Volume 15 in the box with the fan’s order for Volume 14. Doing it this way ensures that only those fans who have legitimately followed through with the series will get J. Michael Straczynski's gift of Volume 15.

We knew that this more secure method meant much more work for us. We'll be verifying everyone twice -- first, when you submit your form, and again within 24 hours of ordering Volume 14 so that we can match your confirmation number to your Volume 14 order.

But what would be easier for us would not be not best for you, the fan. Doing it this way is virtual 100% insurance that the only fans to get Volume 15 will be the fans that have ordered the complete 14-volume set, thereby protecting everyone’s investment.

For this we want to apologize for any confusion, and know that this change was made only because it is better for you.

BEFORE YOU START:
Getting Your Order Numbers

To complete the form, you must have your order numbers for volumes 1 through 13 ready.

STEP 1:
Complete Every Field in the Contact Information Section

This data is critical for validating your Proof of Purchase Form against your order numbers. If this information is not complete, it will void your submission.

STEP 2:
Input Your CafePress Order Numbers Next to Each Volume Number

HELPFUL NOTE: If you ordered more than one volume at the same time, repeat the order number for those volumes.

EXAMPLE: You ordered Volumes 3, 4, and 5 at the same time, and the order number for all these volumes was 46179427. You will input the same order number -- in our example it's 46179427 -- in each of the slots for Volumes 3, 4, and 5, even though it is the same number.

If you received some or all of your books as gifts, if you bought more than one set, or if you have an unusual order number problem or any other question, please see our FAQ below for additional information BEFORE you submit your Proof of Purchase Form.

STEP 3:
Click Submit

After clicking the 'CLICK HERE' submit button, a copy of your Proof of Purchase Form with a confirmation number will appear on your screen. All of the information on this confirmation screen will also be automatically emailed to the email address you provided.

In the unlikely event of any problem with your submission, we highly recommend that you write down this confirmation number or print out the entire confirmation screen.

All submissions will be manually verified. You will get another email from us within 7 business days, letting you know that your submission was approved and that, once you order Volume 14, your free Volume 15 will be included.

FREQUENTLY ASKED QUESTIONS

I'm confused. In the FAQ on the Babylon 5 Scripts website, it specifically states that the Proof of Purchase Form would be available after Volume 14 was released, but now you are asking for it to be filled out BEFORE Volume 14 is released. Please explain.

As we talk about at the top of this page, as the fervor grew for the 14-volume set, in part because of the material being exhumed from J. I have no idea where my order numbers are. I do not keep email copies of receipts and I threw away the paper documentation that came with each book. What do I do now?

You have several choices to get your order numbers:

1) The easiest way to INSTANTLY get ALL your order numbers is to login to your CafePress account. What happens after I complete the Proof of Purchase Form?

After you complete your Proof of Purchase Form, a copy of your Proof of Purchase Form with a confirmation number will appear on your screen. You will also receive a copy of this information by email. No further action is required. Your Volume 15 will automatically be included with your order for Volume 14. When you place your Volume 14 order, we will check purchase validation using your last name before your Volume 14 order is shipped.

Even though you are receiving a confirmation number, you will not actually be using the confirmation number for anything concrete. It is just a fast way for us to be able to look up your Proof of Purchase Form submission. You will not be required to include your confirmation number when you order Volume 14. In other words, the Proof of Purchase Form confirmation number is for internal use only.

Is the Proof of Purchase Form only online? Is there a form I can fill out and mail in?

We are only accepting the Proof of Purchase Form online, electronically.

Some of my personal information has changed since I first started buying the script books. For example, I have a different email address or shipping address. Will this cause any problem for my Proof of Purchase Form?

Not at all. To verify your purchases, all we need are your name and your order numbers. The rest of the information we require is for contact purposes. Since your free Volume 15 is being sent with your Volume 14 order, we are assuming that the address you will use for your Volume 14 order is correct.

I can't find that itty-bitty number that you told us to look for in the beginning. What happened?

After fans informed us that having to track itty-bitty numbers on the back of each volume was problematic, we scrapped that requirement. We announced this by email after Volume 3. Originally, you needed to have your order number and the itty-bitty number on the back of the volume. So we made things easier for everyone by just requiring the order numbers, and not the itty-bitty numbers.

At one point, the proof of purchase process required a tiny code on the back of each volume. Do I still need those codes?

No, you don't. See response above about the itty-bitty numbers.

If I ordered my books by phone, how do I get my order numbers?

It is our understanding that when you order with CafePress by phone, and you use the same email address each time you order, they should have a complete record of all your order numbers.

I can't login to CafePress to get my order numbers. Now what?

I went into my account and only some of my order numbers are listed. I placed some of my orders online, and some of them by phone, and the phone orders are not listed. Yikes. What do I do now?

What if I ordered more than one copy at a time as each book was being released? In other words, every time a volume came out, I ordered two (or more) copies of the same volume at once? How do I fill out the Proof of Purchase Form?

There is a check box on the form exactly for folks like you. We also suggest that you add a comment in the Notes/Comments section of the Proof of Purchase Form as well, indicating the number of copies you ordered every time.

I bought more than one set at different times. Will I need to submit a Proof of Purchase Form for each set?

Yes. It is only when more than one set is purchased at the same time that you will use a single Proof of Purchase Form.

But this is different. Therefore, it is imperative that you indicate in the comments section of the Proof of Purchase Form that this is a second submission for a second set of books.

There was an error/mix-up with one of my orders and I was sent the wrong volume and told to keep it as a gift for the hassle. For example, I ordered Volume 5 but was sent Volume 6. I was then sent the correct volume but had a free volume which I did not buy. How do I handle this on the Proof of Purchase Form?

All you need to do is to use the order number for the correct volume for the volume that was sent to you for free. In other words, if you ordered Volume 5 and got a free Volume 6, you would use the Volume 5 order number as the order number for Volume 6. We have a record of any time this happened, so by entering the same order number for both volumes, we will be able to see which of the volumes was authorized as a comp and validate your Proof of Purchase Form.

I want to order the last few volumes at the same time, in order to save on shipping. For example, I want to order volumes 12, 13 and 14 all at once, but the Proof of Purchase Form asks for the order numbers for volumes 1 through 13. What do I do?

We require that you complete your Proof of Purchase Form before you order Volume 14. This is one of those scenarios where we realize that this could be a problem. So we do have a workaround. If you complete your Proof of Purchase Form within 20 minutes of placing an order that includes a copy of Volume 14, we will be able to match up your Volume 14 order and make sure that it includes J. Michael Straczynski’s gift to you of a free Volume 15.

Specifically, what you need to do is place your order and then immediately go to http://www.Babylon5scripts.com/proof. In the comments area of the Proof of Purchase Form, please indicate to the Scripts Team that you have just placed your order moments before.

Because Volume 15 can only be shipped with Volume 14, if you neglect to complete your Proof of Purchase Form, you won’t get the free Volume 15. We don’t want that to happen. So, please, please, please, please, complete your Proof of Purchase Form immediately after you order Volume 14.

Please note that even though you're purchasing Volume 14 in this example, we’re only asking for the order numbers for volumes 1 through 13.

If I buy a complete set once Volume 14 is released, all 14 volumes at once, do I still need to complete the Proof of Purchase Form?

Yes. You must complete the Proof of Purchase Form within 20 minutes of purchasing the complete set of volumes. We recommend that you go through the CafePress cart, order the books, note the CafePress order number that you will receive after you have submitted your order, and then immediately go to http://www.Babylon5scripts.com/proof to submit your Proof of Purchase Form.

I bought one or more volume(s) on eBay. Can I still get the free Volume 15?

Unfortunately, you can't. Your Proof of Purchase Form requires an order number from CafePress. If you bought a used copy on eBay, then you don’t have a CafePress order number.

One or more of my volume(s) were given to me as gifts. Can I still get the free Volume 15 even though the orders are in different names?

Yes. Get the order number from the people who gifted you the books and put those order numbers in the fields for the volumes you were gifted.

Next, and just as important, in the Notes/Comments field of the Proof of Purchase Form, please let us know which volumes were gifts and the name of the person who gifted it to you. We will then match up that person’s name to that order number and link it back to you.

I know that Volume 15 is free, but what about the shipping and handling?

Volume 15 is a thank you gift from J. Michael Straczynski in appreciation of your commitment to the 14-volume series. Because this is a gift, he is paying for all of the expenses related to your copy of Volume 15. The book and the shipping and handling are 100% covered by J. Michael Straczynski.

Is shipping and handling for Volume 15 covered even for international orders?

Yes. Volume 15 is a gift from J. Michael Straczynski, and part of his gift is the shipping and handling, even for international customers.

If there is a problem with my submission, how will I know?

We will contact you within 72 business hours of your submission if there is any problem. For this reason, it is critical that you include your phone number. We find that approximately 20% of all our emails get caught in spam folders and never reach the fans. Therefore, our only alternative method to contact you is by phone.

I submitted my Proof of Purchase Form, but did not receive the confirmation email. Now what?

As long as you have your confirmation number you are covered, which is why we highly, highly recommend that you write this number down or print out the entire confirmation screen after you submit your form.

After you submit your form, you will get a confirmation screen letting you know that your submission was successfully submitted. What you see on that screen is exactly what would be emailed to you.

If I bring my entire 15-volume set to a convention where JMS is appearing, will he sign them all?

We have been told that Mr. Straczynski will happily, even gleefully, autograph any and all of the script books (as convention schedules allow).

I have a special situation, and was told by Jaclyn, the head of the Scripts Team, that when I submit my Proof of Purchase Form, I should send it to her attention. How do I do that?

The way to submit a Proof of Purchase Form to Jaclyn’s attention is to put “ATTENTION JACLYN” in the Notes/Comments field of the Proof of Purchase Form. Make sure it is the first thing you type in that comment field.

What if I need to change something on a Proof of Purchase Form that I've already submitted?
